Initial referral.

From the initial referral we will gather all necessary information we need from you or the individual referring on your behalf.
At this stage times, prices and detailed requirements will be agreed.

First Visit (Initial Visit)


Once the referral has been confirmed you will be contacted by one of our senior care workers who will arrange to meet you within your home to discuss further your care requirements.

During the first initial visit the senior care worker will complete necessary documents relating to your care package.
You will be able to discuss and agree your care plan with the senior care worker.
The initial visit will take around 1 hour to complete and you will be given a service users guide that is to be kept in your home, this will give you contact details of our out of hours service.

The first initial visit must be completed prior to the commencement of your care package.
In an emergency the first visit must be completed within two days after the commencement of the emergency package.

Co-ordination
Once all your details have been taken the senior care worker will arrange to discuss your needs with the Registered Manager and Co-ordinator.
Your details will be placed into our secure computer system and you will be matched with an available care worker.
Your care package will begin.

1st Week
After the first week of the commencement of care package, the senior care worker will arrange to visit you again to ensure that the care package is working well and to discuss any concerns.

3 Months.
Every three months the senior will visit you in your home and carry out a review of your needs. The senior care worker may have to update your care plan if any changes have become apparent. You will also have the opportunity to let us know if you are happy with the care.

6 months
Every six months a risk assessment of your environment and manual handling needs will be updated. This is to ensure that the care package is being carried out in a safe manner under Health and Safety Laws.

Annually
Every May you will be sent an annual survey. Completion is confidential and at the service users discretion
The Survey will give you the opportunity to tell us how we are doing and if any changes are required.
The results will be collated and sent to the Care Standards inspection and used as a tool to assess the performance in the company.
